All responses and requests have been edited to remove information that might identify the requester. In cases where we feel information might do this, we will not publish the response.

We try to include all of the supporting documents that we supplied in response to each request. In some cases these documents might be too large or held in a format that makes them unsuitable for publication on the website. Contact us if you want to see such supporting information.
